ID PROFORMA NOTICE

Dear Parents,
Your ward has received the ID proforma today. You are requested to fill in your ward’s details in block letters and attach two recent passport-size photographs of your son wearing his school uniform.
- Please ensure that his name is clearly written on the reverse side of the photograph.
- Kindly submit the completed form along with the photograph to your son’s class teacher by Monday, 2nd February 2026.
- Do not fill up the class & section in the proforma.
- Please ensure that all the details provided are accurate, as they will be used to process the Identity Card and the Escort Card for the 2026-27 session.
